Beyond Cortisol: Rethinking Adrenal Care

Adrenal insufficiency is defined by a problem with cortisol—but is cortisol alone enough to explain what living with the condition actually feels like?

In this episode, we step beyond a cortisol-only view of adrenal insufficiency and look at the broader systems involved in keeping the body stable. Cortisol interacts with blood pressure, circulation, glucose regulation, the nervous system, immune and inflammatory responses, sleep, energy, fluid balance, and the body's response to physical and emotional stress.

We explore why replacing a missing hormone is essential, yet cannot perfectly recreate the extraordinarily precise way a healthy HPA axis adjusts cortisol throughout the day. Real life is constantly changing, while replacement therapy has practical limitations.

The discussion also highlights important differences among primary, secondary, tertiary, and steroid-induced adrenal insufficiency. In primary adrenal insufficiency, aldosterone deficiency can add another layer involving sodium, potassium, blood volume, and blood pressure. In central forms of adrenal insufficiency, other pituitary or hormonal issues may sometimes be part of the larger picture.

This broader perspective can also help explain why two people with similar diagnoses—or even similar laboratory results—may experience daily life very differently. Sleep, illness, pain, physical conditioning, medications, nutrition, other health conditions, temperature, stress, and accumulated physiological demands can all influence how someone feels.

Rethinking adrenal care doesn't mean cortisol is unimportant. Cortisol replacement remains central to treating adrenal insufficiency. It means recognizing that good care also involves understanding the person and the interconnected systems surrounding that cortisol deficiency.
