Why can blood pressure be so misleading in adrenal insufficiency?

In this episode, we explore why the familiar idea that adrenal insufficiency always means low blood pressure doesn't tell the whole story. Blood pressure is dynamic, influenced by multiple systems, and a single reading may not reflect what your body is experiencing throughout the day.

We look at the relationship between cortisol, aldosterone, blood volume, vascular tone, the nervous system, and adrenaline. In primary adrenal insufficiency, or Addison's disease, loss of aldosterone can contribute to sodium loss, reduced blood volume, and lower blood pressure. But that does not mean everyone with adrenal insufficiency will consistently have low readings.

We also explore why blood pressure may fluctuate, why some people experience temporary increases or adrenaline-driven surges, and how the body's compensatory mechanisms can sometimes make a reading look surprisingly normal—or even high—while the person still feels profoundly unwell.

This episode challenges the idea that one blood pressure number can tell us whether someone is adequately supported or whether their symptoms are related to adrenal insufficiency. Instead, we focus on patterns, changes from an individual's usual baseline, accompanying symptoms, and the broader clinical picture.

Most importantly, high or normal blood pressure should not automatically be used to dismiss adrenal insufficiency, while every unusual blood pressure reading should not automatically be blamed on it either. Other cardiovascular, endocrine, medication-related, and health factors still matter.
